What our public website forms collect
Our consultation form collects only what is needed to schedule and prepare for a conversation: your name, professional credentials, email address, phone number, current state of residence, the states where you need support, the type of service needed, your career stage, your desired timeline, your preferred contact method, and any general comments you choose to provide. Optional fields cover specialty, license status, number of states, employer or practice type, best time to contact, and a general question.
Never submit these through a public form or standard email
- Passwords of any kind
- Social Security numbers
- State-board portal credentials
- DEA credentials or registration numbers
- Financial account or payment card information
- Detailed disciplinary, malpractice, or health information
- Full images of government identification
Credentials
We never request your board credentials
Brya Services Fast Licensed does not request, require, or accept state-board portal credentials or passwords. No member of our licensure team will ever ask you for a password. Any message claiming otherwise — even one that appears to come from us — should be treated as suspicious and reported.
Secure document transfer: documents should be exchanged only through a verified secure, encrypted, access-controlled portal. Until a secure portal is confirmed as available to you in writing, do not send licensure documents by email or through this website. This site does not offer public document uploads.
Access to information you share with us is limited to the members of our licensure team who need it to support your applications, and we keep records only as long as needed to provide services and meet recordkeeping obligations.
